一个年轻的日本女子，伽椰子的同母的妹妹知道该如何阻止伽椰子的怨念继续蔓延，前往上集发生惨剧的芝加哥的公寓，想永远消除伽椰子的诅咒并拯救公寓里的人。 医院里上集的杰克被伽椰子折断胫骨死去。而他的主治医生来到芝加哥进入咒怨小屋，最后也在医院里被伽椰子追杀。 葛丽琴在家中画画，画完后去洗毛笔听见奇怪声音，赶去一看，自己的画倒在了红色颜料上。拿起一看，眼镜等部位都被弄出个洞。画突然变成伽椰子，随即出现红色手印。后被马克思发现和画中死的一样。 马克思的老板上了汽车，发现开动不了，修好车发现骏雄在副驾驶位上。 伽椰子的妹妹来到芝加哥准备救助马克思一家，后被狂暴马克思所杀，变成新的咒怨，杀死了马克思。 最后罗斯喝下伽椰子的血，拯救了姐姐。与此同时，杀了伽椰子妹妹的马克思使其产生新的咒怨以致自己的死亡。画面最后，姐姐抱着罗斯，殊不知罗斯已变成伽椰子。
